2009 Season Registration Requirements
Football players and cheerleaders will not be able to begin practice until all paperwork has been turned in!!! The following documents are needed before the start the season for your child to take the practice field:
1. Sports physical signed and dated by the child's physician in 2009. Physicals are good for 2 years, but must be signed and dated by the physician each year.
2. FINAL 2008 - 2009 Report Card - All Pages
3. Registration form - all pages, signed and dated
4. Birth Certificate - please note that if your child's squad advances past the State Competition, an original birth certificate will be required for age certification. This is a Pop Warner national rule, not a Youngsville rule. Please make sure you have this document readily available!!!!!
